Please contact the Hultquist Foundation, submit four copies of the written request and supporting documentation (one copy of most recent Form 990 and audited financial statements) by mail or delivery.

First time applicants must also provide a copy of their IRS 501(c)(3) determination letter.

• Request Range: $5,000- $300,000 (average award $45,000)
